Independent Living For Seniors --- Your Stay At Home Choice!

ILS PACE Day Center

Most care is provided in a noninstitutional setting - in the day center and/or your home. Our center is located on Hudson Avenue in Irondequoit. Independent Living For Seniors participants attend the adult center where their care team is located. Day care attendance is determined based on individual needs.

The activities offered in the center range from exercise programs, crafts, cooking, gardening and spiritual groups, to pet therapy, visits to other centers, outings for lunch, and entertainment. The ILS Day Center is also where you receive medical and rehabilitation services and a hot meal.

Our care teams coordinate transportation to medical appointments and the adult center. In addition, the program provides needed medical attention, medical equipment and supplies. Alternative Care Settings: ILS has two Alternative Care Settings. They are located at:

The members of your care team, the interdisciplinary team, work with you to determine if this alternative setting is an appropriate service for you. The Alternative Care Setting provides recreation and activity opportunities in addition to what is offered at the PACE day center. When necessary, services may also be provided in your home, in a hospital or in a nursing home. We have contracts with physician specialists (such as cardiologists, urologists and orthopedists), laboratory and x-ray services, and you may receive services at these offices.
